Add Pages to Passport

Global Allianz Immigration consultants will prepare and file all the required forms and documents for adding pages to the passport.

If you need to add pages to your passport book the appropriate form to use is the DS-4085 add pages application form. Adding pages to passport is less complicated then getting a new or renewal passport. It asks for your name, social security number, passport back information, and to sign your application. The add pages application form does not require a passport photo. If your passport is still valid, and in good condition, but you have run out of pages for visas and entry/exit stamps, there is no need to obtain a whole new passport. Additional pages can normally be added twice to a passport book before the book must be replaced. Additional pages will not be added to passports which are in poor condition, damaged or mutilated.

The following are required to add pages to your Passport:

1. DS-4085 – Application for Additional Visa Pages.

2. Current U.S. passport.

3. Self-addressed stamped envelope.
