Name Change

Global Allianz Immigration consultants will prepare and file all the required forms and documents for changing the name on passport.

A U.S. passport allows U.S. citizens to travel outside the United States. In addition, a U.S. passport is also recognized as a document proving one’s U.S. citizenship as well as identity. Therefore, keeping your U.S. passport current is very important, especially if you need to change name on the passport or if you are in the process of changing name on the passport there are at least three scenarios when changing name on a passport.

Scenario 1: If your current U.S. passport is less than 1 year old AND you need to change name on the passport because it was legally changed because of marriage or court order.

You must include the following in your application package and mail it to the address listed on Form DS-5504:

1. Accurately completed Form DS-5504, Application for a U.S. Passport: Name Change, Data Correction, and Limited Passport Book Replacement.

2. Your current, valid U.S. passport.

3. An original or certified copy of your marriage certificate, or a government-issued document proving that your legal name has changed; and

4. Two recent color passport photos

Scenario 2: If your current U.S. passport is more than 1 year old and current AND you need to change name on the passport because it was legally changed because of marriage or court order.

You must include the following in your application package and mail it to the address listed on Form DS-82:

1. Accurately completed Form DS-82, Application for a U.S. Passport by Mail and

2. Your current, valid U.S. passport.

3. An original or certified copy of your marriage certificate, or a government-issued document proving that your legal name has changed.

4. Two recent color passport photos; and

5. The appropriate filing fee.

Scenario 3: If you cannot document your name change through a marriage certificate or court order OR if you are under the age of 16 and your passport was issued more than a year ago.

You must submit the following at an Acceptance Facility or a Passport Agency IN PERSON:

1. Accurately completed Form DS-11, Application for a U.S. Passport and

2. Your current, valid U.S. passport.

3. At least 3 public record documents that show your date and place of birth and that also show the exclusive use of you assumed name for at least 5 years.

4. Two recent color passport photos; and

5. The appropriate filing fee.
